Garuda Indonesia Flight 865 was a scheduled international flight from Fukuoka, Japan, to Jakarta, Indonesia. On June 13, 1996, the crew of the aircraft, a McDonnell Douglas DC-10-30, attempted to abort take-off after reaching a speed above V₁. It happened because when the aircraft reached rotate speed, the fan blade from engine number three separated. When the crew tried to stop it, the aircraft slid into a threshold, which severed fuel line number three and the landing gear, causing the aircraft to burst into flames. Three out of 275 aboard were killed. The plane was written off.
